我清楚了解下列事项：
|

1. |
我将自行负责在中国申请国际护照。 |
|
2. |
若我的学生签证申请被马来西亚有关政府部门拒绝，学院将退还三千元人民币之申请费于我。
|
|
3. |
倘若我已获得马来西亚的学生签证，并完成在英迪学院的课程，学院将协助我向有关属意国家使馆申请升学签证但英迪学院不须对该签证的申请结果负任何责任。 |

I understand that:
|
|
1. |
I will be solely responsible for the application of my own
international passport in China. |
|
2. |
If my application for student visa is rejected by
Malaysian authorities, RMB 3,000 shall be refunded to me without interest. |
|
3. |
In the event that I have been granted visa to study in
Malaysia and have completed my studies at INTI International University, INTI International University will
assist me in my application for the necessary visa from the foreign embassy of
the country selected by me to further studies. I will not hold INTI International University
liable in the event that the relevant foreign embassy rejects my application
for student visa to that country. |

To uphold the
integrity and good name of INTI International University, I agree to abide by the INTI International University
Code of Honour, as outlined in the General Rules and Regulations in the INTI
International University catalogue. At no time, will I be engaged in: |

Lying |

偷窃
Stealing |
|
3. |
剽窃
Plagiarism |
|
4. |
在考试或是作业中作弊
Cheating in examinations, tests, written assignments or any academic
work |

5. |
抽烟/酗酒/吸毒
Smoking / Consuming alcoholic beverages / Using drugs |
|
6. |
破坏公物
Vandalism |
|
7. |
扰乱院方活动
Disruption of activities |
|
8. |
猥亵或是不规矩的行为
Obscene or disorderly conduct |
|
9. |
非法拥有或使用危险武器
Possession and use of dangerous weapons |
|
10. |
连续无故缺课
Consistent non-attendance of classes |
|
11. |
根据私立高等教育法令，学生严禁参加任何政治团体以及违法的社会组织等。
Under the Private Higher Education Act, students are prohibited from
being associated with political party, unlawful society, etc. |

我，谨此签署宣誓，以上填写之资料皆属真实无误，并同意遵守英迪大学守则。我了解任何虚报或隐瞒行为将会影响院方对我申请入学的核准。
I, the undersigned, declare that the information on this application
form is accurate and true and I agree to abide by the rules and regulations of
INTI International University. I understand that any false information given or withheld will
affect the University's decision on my application. |
